Under-Eye Bags That Won't Shift? When Lower Eyelid Blepharoplasty Makes Sense

Under-Eye Bags That Won't Shift? When Lower Eyelid Blepharoplasty Makes Sense

30 Sep 2026

Under-eye bags caused by fat that has pushed forward won't respond to creams, sleep or cold spoons. When that's the cause, surgery is often the only thing that deals with the problem directly.

You've tried eye creams, more sleep and cutting back on salt, and the bags are still there every morning. For many people that's the point where they start looking into lower eyelid blepharoplasty. It's one of the most requested forms of cosmetic eyelid surgery, but it isn't the right answer for everyone, so it's worth understanding when it genuinely makes sense.

Why do under-eye bags form?

With age, the thin tissue that holds fat in place around the eye weakens. The fat bulges forward and creates the puffiness and tired look most people first notice in photos. Skin also loses elasticity over time, which can add loose or excess skin to the picture. Genetics play a big part too, which is why some people have noticeable bags in their twenties.

Not all puffiness is fat, though. Fluid retention from poor sleep, allergies, salty food or alcohol causes swelling that comes and goes. That type doesn't usually need surgery, and it's the single most common reason we tell someone not to have it.

What is lower eyelid blepharoplasty?

Lower eyelid blepharoplasty removes or repositions the fat causing under-eye bags and, where needed, tightens or removes a small amount of excess skin. The aim is a smoother, fresher lower lid that still looks like you.

Depending on your anatomy, your surgeon may recommend one of two approaches:

  • Transconjunctival blepharoplasty. The incision is made on the inside of the lower lid, so there's no visible scar on the skin. Usually suited to people whose main concern is fat rather than loose skin.
  • Transcutaneous approach. A fine incision just below the lash line, which allows excess skin to be addressed as well. The scar sits in a natural crease and becomes hard to notice once healed.

In some cases fat is repositioned to fill a hollow tear trough rather than simply removed, which softens the line between the lower lid and the cheek.

When lower eyelid blepharoplasty makes sense

Lower eyelid blepharoplasty tends to suit people who:

  • Have under-eye bags present every day, not just after a late night
  • Have tried non-surgical options without a lasting result
  • Have bulging fat, loose skin or both
  • Are in good general health with healthy eyes
  • Have realistic expectations about what surgery can and can't change

When another option may suit you better

Surgery isn't always the right first step. Something else may be more suitable if:

  • Your puffiness changes a lot from day to day, which points to fluid rather than fat
  • The main issue is hollowness, where dermal fillers may be considered instead
  • Your dark circles come from pigmentation or thin skin rather than bags
  • You have significant dry eye or another eye condition that needs treating first

A proper assessment is the only reliable way to tell these apart.

What happens on the day?

Lower lid surgery is a day case, so you go home the same day. It's commonly performed under local anaesthetic, sometimes with sedation, and typically takes around one to two hours. Your surgeon talks you through the plan, the approach being used and your aftercare before anything begins.

What is recovery like?

  • Bruising and swelling are normal and usually settle over one to two weeks
  • Cold compresses and keeping your head raised help in the first few days
  • Any skin stitches are usually removed within the first week
  • Many people return to desk-based work after around 10 to 14 days
  • Strenuous exercise, swimming and eye make-up wait until your surgeon says so

The final result becomes clearer over several weeks as the remaining swelling settles.

Are there any risks?

As with any surgery, lower eyelid blepharoplasty carries some risk. That includes temporary dry or watery eyes, infection, visible scarring, slight asymmetry or a change in the position of the lower lid. Serious complications are uncommon, and choosing an experienced eyelid surgeon, then following your aftercare advice, keeps the risk as low as it can be.

How long do the results last?

Surgical eye bag removal gives long-lasting results, and for many patients the fat doesn't return in the same way. Your skin will still age naturally, so some change over the years is normal. That's very different from dermal fillers, which need topping up.

What lower eyelid blepharoplasty costs at Optimal Vision

Lower lid blepharoplasty is commonly performed on both eyelids together, and at Optimal Vision that's priced from £3,900, usually £5,000. Upper lid blepharoplasty is priced separately, from £3,250 for both upper lids. A combined upper and lower procedure covering all four eyelids is £7,000. These are starting prices, and your exact quote is confirmed at consultation once your surgeon has assessed your eyelids and agreed the approach with you.

Why the assessment matters

The success of this procedure depends on choosing the right approach for your face. Your consultation includes a close look at your lower lids, skin quality, eye health and what you want to change, with Dr Lisa Jagan, our fellowship-trained oculoplastic surgeon. If surgery isn't the best route for you, she'll tell you and explain the alternatives. If the heaviness turns out to be a drooping lid margin rather than skin or fat, ptosis repair is a different operation entirely.

Ready to take the next step?

Frequently asked questions

Is lower eyelid blepharoplasty painful?

The procedure is performed under anaesthetic, so you shouldn't feel pain during surgery. Mild discomfort, tightness and swelling afterwards are common and manageable with simple pain relief.

Will I have a visible scar?

With the transconjunctival approach the incision is inside the lid, so there's no external scar. With the transcutaneous approach the fine scar sits just below the lashes and usually fades well.

How long before I look presentable again?

Most bruising and swelling settles within one to two weeks, and many people feel comfortable going out after that. The full result develops over the following weeks.

Can fillers remove under-eye bags instead?

Dermal fillers soften a hollow tear trough, but they don't remove bulging fat. Where fat is the cause, lower eyelid blepharoplasty is the more effective option.

Am I suitable for lower eyelid blepharoplasty?

Suitability depends on the cause of your under-eye bags, your skin, your eye health and your expectations. A face-to-face consultation is the only way to find out.
